شروط استيراد المستلزمات الطبية المعقمة
  • فاتورة اصلية عليها اسماء الاصناف (FULL DESCRIPTION +CATALOG NB) واسم المصنع
  • AUTHORIZATION LETTER FROM MANUFACTURER TO THE ENTITY WHICH ISSUED THE INVOICE
  • شهادة تعقيم عليها رقم الفاتورة  رقم الطبخة وانتهاء الصلاحية .
  • تسجيل مسبق للممستلزمات الطبية المغروسة في دائرة البرامج والمشاريع
  • CE VALID OR EU MDR CERTIFICATE OR CE EXTENSION FILE  FOR THE MANUFACTURER OR CERTIFICATE OF FOREIGN GOVERNMENT (USFDA)
  • افادة مطابقة من معهد البحوث
  • بيان جمركي                
  • الاستمارة الخاصة بال RECALL/SAFETY NOTICE
  • تعهد بيع حسب سقوف الارباح المحددة في القرار 1/1342 (المستلزمات الغير مغروسة) و1/1314 (للمغروسات)
 
شروط استيراد الكواشف المخبريه
  • فاتورة اصلية عليها اسماء الاصناف (FULL DESCRIPTION +CATALOG NB) واسم المصنع
  • AUTHORIZATION LETTER FROM MANUFACTURER TO THE ENTITY WHICH ISSUED THE INVOICE
  • PACKING LIST عليها رقم الفاتورة  رقم الطبخة وانتهاء الصلاحية .
  • CE VALID OR EU MDR CERTIFICATE OR CE EXTENSION FILE  FOR THE MANUFACTURER  FOR IVDS AND RAPID TESTS USED TO DIAGNOSE A DISEASE
  • ISO 113485  OR CERTIFICATE OF FOREIGN GOVERNMENT (USFDA)
  • بيان جمركي
  • الاستمارة الخاصة بال RECALL/SAFETY NOTICE
  • تعهد بيع حسب سقوف الارباح المحددة في القرار 1/1342
