An authorized free sale certificate or CFG describing the classification of the product in its country of origin (medical devices.._) with the manufacturer and product name , stamped from :

Chamber of commerce in the country of origin
The Lebanese embassy in the country of origin

ISO 1385
EU MDR certificates OR CE extended (file to be submitted at diwan )
Methods of sterilization
2 samples of the product in their final form as they are going to be sold in the market or mock up
A long term stability data on one batch number for the whole product shelf-life mentioning batch size, manufacturer data and batch number (for solutions only )
Storage conditions
Summary of product characteristics
