MOH requirements for registration of contact lenses /solution registration
- An authorized free sale certificate or CFG describing the classification of the product in its country of origin (medical devices.._) with the manufacturer and product name , stamped from :
- Chamber of commerce in the country of origin
- The Lebanese embassy in the country of origin
- ISO 1385
- EU MDR certificates OR CE extended (file to be submitted at diwan )
- Methods of sterilization
- 2 samples of the product in their final form as they are going to be sold in the market or mock up
- A long term stability data on one batch number for the whole product shelf-life mentioning batch size, manufacturer data and batch number (for solutions only )
- Storage conditions
- Summary of product characteristics
MOH requirements for importation of contact lenses /solution registration
Same requirements of importation of sterile items.
